Class Registration & Liability Waiver

PLEASE READ & SIGN:

I am aware that the staff at Happy Little Flame uses caution and care, but cannot protect participants from sudden, unforeseen situations that are out of their control. In the event of injury or illness during the class, I authorize emergency medical care or transportation should it be needed.

With my signature, I release and hold harmless Happy Little Flame and it's owners, members, and staff, and all other persons and entities associated with Happy Little Flame, from any and all injuries or damage from any claims or causes of action, for any loss or injury suffered by the participant and his/her friends and families while on the premises.

By signing this document, I also agree to respect the property, equipment, materials, staff, and other participants throughout the duration of my visit at Happy Little Flame. I understand that if I violate this agreement, there may be consequences that could result in legal action.

I grant full permission to Happy Little Flame to use my/the participant’s photograph in videos, publications, marketing materials, and any other records of events.

I have read and fully understand the above statements, and certify that, as the participant or parent/guardian of the participant; all registration and release information provided is true.
